The Honorable                                                                          , of Sacramento County Superior Court, Department                     , hereby authorizes DRUG DETECTION LABORATORIES, INC. to provide the "RANDOM CALL" drug testing services for the individual and /or parties stated above.  The RANDOM CALL service may be provided up to an average of                  times per month beginning and ending on the dates stated above.  The contacted party will be allowed                   hours to respond to the contact and provide a urine sample for drug testing.  Drug testing is to be done according to the "RANDOM CALL DRUG TESTING PROTOCOL" (utilizing "zero tolerance" testing) of Drug Detection Laboratories, Inc.   The reports and any infortmation in regards to making contact, including "NO SHOW" documentation is to be provided to this court and/or the following individuals.
